A backrest that is only slightly too wide, too tall or positioned poorly can affect far more than comfort. It can change how easily a person reaches their wheels, how stable they feel when moving, and whether their cushion and posture supports can do their job. Knowing how to size wheelchair backrest options starts with the person’s seated posture, not simply the measurements printed on the wheelchair frame.
A wheelchair back is part of a seating system. For some users, a basic tension-adjustable backrest provides enough support. Others may need a contoured rigid back with lateral support, carefully selected with an occupational therapist or physiotherapist. The right approach depends on the user’s diagnosis, muscle control, skin integrity, daily activities and transfer needs.
Start with a proper seated assessment
Measurements should be taken while the wheelchair user is sitting as close as possible to their usual functional posture. Their pelvis should be positioned back in the chair, feet supported, and usual cushion in place. Measuring someone while they are perched forward, sliding down the cushion or sitting on a different surface can produce an inaccurate result.
If the user has a changing posture, significant scoliosis, pelvic asymmetry, reduced trunk control, pain, pressure injuries or a recent change in function, seek advice from a qualified seating clinician before choosing a backrest. These situations often require more than a standard width and height measurement. A clinical assessment can identify where support is needed and where clearance is equally important.
How to size wheelchair backrest width
Backrest width is generally based on the wheelchair’s usable seat width, but it should not be assumed that every back will fit every chair. Measure the distance between the wheelchair’s side frames or armrest supports at the point where the backrest mounting hardware will sit. Check the manufacturer’s specification for whether the stated back width refers to the shell width, mounting width or recommended chair width.
A rigid backrest is commonly selected close to the chair’s seat width. It should support the trunk without pressing into the hips, upper arms or ribs. A back that is too narrow can feel restrictive, create pressure at the sides and interfere with clothing or transfers. One that is too wide may allow the user to lean or collapse sideways, particularly if they have limited trunk control.
The relationship between the backrest and the wheels also matters. Active wheelchair users often need clear access to the push rims for efficient propulsion. A wide back shell or protruding mounting hardware can make that reach less natural. On the other hand, a user who requires lateral trunk support may benefit from a more encompassing configuration, even if it slightly changes access to the wheels. That trade-off should be considered in the context of how the chair is used every day.
Allow for clothing, posture and hardware
Do not size solely around bare-body measurements. Winter clothing, continence products, braces and changes in body shape can alter the available space. Also inspect where the backrest brackets will mount. Wheelchairs with curved back canes, folding mechanisms, push handles or rear wheel adjustments may have fitting limitations.
A compatible backrest should mount securely without preventing the chair from folding, if folding is required. It should also leave enough room for any headrest, lateral supports, pelvic positioning belt, backpack or anti-tips already used on the chair.
Choose the backrest height for the support required
Height is measured from the top of the cushion to the top edge of the backrest, not from the floor. This distinction is essential because cushions vary considerably in thickness and can compress under load. Measure with the usual cushion fitted and the user seated in their intended position.
Lower backrests are often preferred by active users who need maximum shoulder movement for self-propulsion, transfers, sport or reaching tasks. A low back can provide pelvic and lower trunk support while keeping the upper back and shoulder blades clear. This can be a practical choice for someone with good sitting balance and trunk strength.
Mid-height backs offer more support through the thoracic area and suit many everyday manual and power wheelchair users. They can assist with upright sitting while still allowing useful arm movement. Higher backs provide greater upper trunk support and may be suitable for users with reduced endurance or limited trunk control. They may also form part of a more complex seating arrangement with head support.
The highest available backrest is not automatically the most supportive option. If the top edge sits too high, it can restrict shoulder blade movement, make pushing harder and encourage a rounded posture. If it is too low, the user may fatigue, lean backwards or sideways, or lack the stability needed for safe daily tasks.
Match contour and support to the user
A flat upholstery backrest gives broad, flexible support and can suit users whose posture is stable and whose needs are straightforward. Tension-adjustable models allow straps to be altered to create more shape around the pelvis and trunk. They are useful when moderate positioning changes are needed, although adjustments should be made carefully to avoid creating uncomfortable pressure points.
Contoured rigid backs provide a firmer, more consistent surface. Their shape can encourage a more stable pelvic and trunk position, especially when paired with suitable mounting hardware and a pressure care cushion. Some backs offer adjustable height, angle, depth and lateral positioning, which can help fine-tune support.
Contour should follow the person’s body rather than force their body into a shape. A user with a fixed spinal curve or asymmetrical posture may require customised positioning. Trying to correct a fixed posture with a standard backrest can cause pain, skin pressure and loss of balance. In these cases, clinician-guided seating is the safer option.
Check back angle and seat depth together
Backrest sizing is not only about width and height. Back angle affects pelvic position, stability and comfort. An upright angle may help a person complete tasks at a table or propel efficiently, while a slightly reclined angle may improve tolerance for someone who fatigues or needs more trunk contact. Excessive recline without appropriate pelvic support can lead to sliding forward and increased shear on the skin.
Seat depth also changes how the backrest feels. If the seat is too deep, the user may not be able to sit back against the support without pressure behind the knees. If it is too short, thigh support is reduced and posture may become less stable. Check that there is a small clearance between the front edge of the cushion and the back of the knees, while the pelvis remains properly supported at the rear.
A well-sized backrest works with the cushion, footplates and pelvic support. Adjusting one component often changes the performance of the others.
Measure before ordering or replacing a backrest
Before purchasing, record the wheelchair make and model, seat width, back cane diameter and whether the chair folds. Note the current cushion type and compressed height, the existing backrest height, and any accessories attached near the back canes. Photographs of the rear and side of the chair can also help confirm mounting compatibility when seeking product advice.
For a replacement backrest, do not assume the existing one was correctly sized. Ask the user what is working and what is not. Are they developing discomfort after an hour? Do they struggle to reach the wheels? Is the back too soft, too low or difficult to adjust? Practical feedback is often as valuable as the tape measure.
Complete a real-world comfort check
Once fitted, assess the backrest during normal use rather than only while stationary. The user should be able to sit with their pelvis supported, breathe freely, reach the wheels or controls, and complete typical transfers and activities. Check for rubbing at the hips, shoulder blades and sides of the trunk, particularly after longer periods in the chair.
Look for changes in skin colour, persistent marks, pain, sliding or increased fatigue. Any of these can indicate that the setup needs review. Make adjustments one at a time where possible, then allow the user enough time to feel the difference.
